Precision Epilepsy PLLC is an innovative multi-state tele-epilepsy practice with no in-person clinics available. Dr Millichap is currently licensed and able to provide virtual consultations for patients physically located in any State colored purple in the image below. New States are added regularly. Telehealth Appointments
Appointments are made online. Please register for the Patient Portal and select the desired date and time.
We understand occasional circumstances prevent you from keeping your scheduled appointment. If possible, please notify our office in advance if you are running late or unable to make your appointment. Our scheduling coordinators will do their best to re-schedule you promptly.
For urgent issues, contact your local treating doctor. In case of emergency, call 911 or go to the nearest emergency room.
Insurance and Billing Policy
State Medicaid Accepted
- Medicaid accepted for all States.
- Please provide full Medicaid insurance information during registration process.
- Please enter a note on appointment request indicating State Medicaid Insurance.
Out-of-Network Insurance (Self-Pay)
- Due to the nationwide scope of the practice it is not feasible to enroll "in-network" with the various commercial insurance companies. Dr Millichap is currently considered "out-of-network" for all insurance carriers.
- Patients with commercial insurance should select "Self Pay" during registration process on the Patient Portal.
- The current self-pay fee is $225.00. Payment in full is expected from you at the time of your visit. We will supply you with an invoice that you can submit to your insurance for reimbursement.
- Please contact your insurance ahead of your appointment to learn more about out-of-network visits reimbursement and the potential need for pre-approval or referrals.
Payment Options
Billing and payment is conducted through the Patient Portal. We also offer a flexible payment plan in case of hardship related to the flat fee. Please contact us at [email protected] to inquire.
